Welcome! Let us begin by telling you we sincerely look forward to our first session together.
Every client and every session offers us a new experience, lesson or solution toward a better massage practice. Someone once said that no touch is so light as to leave no mark behind. We know it is a significant act of trust to allow us to work with you and we strive to live up to that responsibility.
As part of our initial session, we will need to gather your medical history and other useful information to develop the best personal treatment plan for you possible. We are health care professionals and we take that responsibility seriously.
Please download the appropriate intake form below, fill it out to the best of your ability, and have it with you for your first appointment. This will minimize time spent on paperwork and maximize time spent on your treatment. Transgender clients please choose the form you feel best represents your body for the purposes of massage and medical history.
If you are not able to complete the form ahead of time, no worries. Just plan for an extra 15 - 20 minutes during your session.
